​Health Improvement Programs

KelseyCare Advantage offers health management programs designed to improve the health of individuals who live with multiple or sufficiently severe chronic conditions such as Diabetes, Congestive Heart Failure or other complicated disease.  Case Managers work with the treating physicians to implement individualized care plans to help support the member and work on self-management techniques.


Enrollment in Health Improvement Programs
If you believe you have a chronic condition that qualifies you for one or more of the Health Improvement Programs listed, please discuss this with your Kelsey-Seybold physician.  Your doctor can refer you for participation in one or more of these programs.


Health Improvement Programs
KelseyCare Advantage offers health management programs designed to improve the health of individuals who live with multiple or sufficiently severe chronic conditions such as Diabetes, Congestive Heart Failure or other complicated disease.  Case Managers work with the treating physicians to implement individualized care plans to help support the member and work on self-management techniques.


Diabetes Management
A member diagnosed with diabetes may be referred by their doctor to Diabetes Education Classes, a Certified Diabetes Educator, and / or a Registered Dietitian.  These classes and health care professionals provide an overview of diabetes, monitoring blood sugars, taking medications, meal planning and exercise to help members minimize or avoid diabetes-related complications.  The information and coaching provided through the Diabetes Management Program will help members gain the information they need to make important lifestyle changes.


Cholesterol Management
The Cholesterol Management Program is designed to aggressively manage unhealthy cholesterol levels in certain patients.  The program is successful in preventing further disease progression in patients with coronary artery disease or other risk factors.  Members enrolled in the Cholesterol Management Program will receive reminder calls or emails when lab tests or other follow-up needs to be scheduled.   Members may ask their Kelsey-Seybold cardiologist for more information about this program.


Asthma and COPD Management
Asthma and COPD affects at least five percent of our population.   A member diagnosed with these conditions may be enrolled in this program and managed by a participating physician, if they have mild symptoms or be followed by a Pulmonologist for more severe progression of the chronic condition. Management of these chronic conditions includes medication monitoring and education, lung function measurement and ongoing coaching regarding risk factors.
